Overview

HealthDrive is seeking a compassionate and reliable Patient Care Technician (PCT) to support our clinical partners across Connecticut.  This mobile position plays a key role in gathering accurate, timely patient vital signs in skilled nursing facilities - helping our partner facilities enhance resident care, improve clinical decision-making, and streamline documentation.  As a Patient Care Technician with HealthDrive, you will travel between 10 - 15 skilled nursing facilities within your regional area, typically visiting 1 - 2 facilities per day.  Using a specialized vitals-collection devine, you will go room to room to collect essential patient health information.   This device automatically uploads and bills for services, ensuring accuracy, compliance, and efficiency for both you and the facility.

Responsibilities

Travel to assigned skilled nursing facilities within your designated Connecticut region (10 - 15 facilities total).

Visit 1 - 2 facilities per day according to a pre-planned schedule.

Collect patient vital signs with accuracy, consistency, and professionalism, using the company-provided equipment. 

Upload patient vial data via the automated system to support timely documentation and billing. 

Interact warmly and respectfully with residents, family members, nursing staff, and facility leadership. 

Maintain a high level of customer service and communication when working withing partner facilities. 

Follow all infection control, privacy, and complaince procedures in accordance with facility and HealthDrive protocols. 

Report equipment issues, scheduling concerns, or unusual patient findings promptly to the appropriate HealthDrive contact. 

 

Qualifications

Certified Patient Care Technician (PCT) or Certified Nursing Assistant (CNA)

Previous experience working in a skilled nursing facility (SNF). 

Demonstrated experience in accurately taking and recording vital signs. 

Ability to travel daily to multiple facilities within the Hartford, Connecticut region; reliable transportation required. 

Strong interpersonal skills with the ability to interact positively with residents, families, and staff. 

Comfortable working independently and managing daily schedules across multiple locations. 

Dependability, professionalism, and a strong sense of empathy and service.
