HealthDrive delivers primary/post-acute care to residents in long-term care, skilled nursing and assisted living facilities. We are seeking a highly skilled and collaborative Primary Care Physician to join our growing telehealth team supporting patients in skilled nursing facilities (SNFs). This is a unique opportunity to deliver high-impact care remotely while partnering closely with on-site clinical teams and leveraging innovative diagnostic technology. In this role, you will conduct on-demand telemedicine visits, interpret diagnostic testing results, and guide care plans in collaboration with facility staff. You will also play a key role in clinical oversight by performing structured evaluations of nurse practitioners to ensure high-quality, patient-centered care.

Work Environment

This is a fully remote position. Physicians will interact with patients and care teams via secure telehealth platforms while supporting facilities equipped with mobile diagnostic technology.

Responsibilities

 

Telehealth Clinical Care

  • Conduct remote primary care and consultative visits with patients in skilled nursing facilities via telemedicine
  • Review and interpret diagnostic testing results obtained through mobile clinical technology (e.g., diagnostic carts)
  • Develop and communicate treatment recommendations in collaboration with on-site medical staff
  • Partner with interdisciplinary care teams to ensure continuity and quality of care
  • Document all patient encounters and recommendations in a timely and compliant manner

Clinical Oversight & Nurse Practitioner Evaluation

 

  • Perform 5–10 structured clinical assessments per day (on designated days) for nurse practitioners
  • Conduct comprehensive chart reviews and clinical audits
  • Observe patient interactions and care delivery (virtually)
  • Provide constructive feedback, coaching, and guidance to support clinical excellence
  • Ensure adherence to evidence-based practices, organizational standards, and regulatory requirements

Qualifications

  • MD or DO degree from an accredited medical school
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Licensed or willing to obtain licensure in New Jersey and Rhode Island; Connecticut licensure may be required in the future
  • Experience working with geriatric populations and/or in skilled nursing facilities strongly preferred
  • Prior telehealth experience is a plus
  • Demonstrated ability to collaborate effectively with interdisciplinary teams
  • Strong clinical judgment and comfort interpreting diagnostic data remotely
